CareLink Community Support Services has an openings for full-time, benefits eligible, Program Supervisor at our Transistion Age program located in West Chester, PA. For over 60 years, CareLink Community Support Services has been dedicated to helping people with disabilities to live in the community with dignity and independence. Our rehabilitation services and professional staff provide the supports that people with serious mental illness, physical disabilities and other developmental challenges need to be active, productive members of their communities.

The Program Superviosr is responsible for the daily operation of the Residential Transitional Age program as well as the implementation of agency mission, values, and goals. Individuals working in this capacity are accountable for staff supervision, implementing quality rehabilitative programming and ensuring the health and welfare of clients. Program Supervisors are part of the Management Team and work closely with CareLink administration to implement agency policies, procedures, and standards of care.

 This position is responsible for supporting community integration. The goal of this position is to promote the pursuit of independence for all individuals participating in service. The essential functions are established based upon the agency mission, values, and goals.

Available Shift:
  • Monday through Friday, 9:00 AM - 5:00 PM

Responsibilities Include, but are not limited to the following:
  • Plan the daily operation of the program.

  • Report all relevant problems and changes to the appropriate CareLink administrative personnel.
  • Assist staff in the planning, coordination, implementation of the day, evening, and/or weekend social and recreational activities for clients.
  • Oversee referrals to the program.
  • Coordinate and develop individual service plans designed to improve the social and self-care skills of clients.
  • Monitor participant medications.
  • Oversee participant’s special medication needs.
  • Oversee menu planning and food ordering on a weekly basis.
  • Ensure coverage for each shift.
  • Work additional shifts when needed to assure proper coverage.
  • Participate in the daily milieu of the program.
  • Demonstrate respect for participants, families, staff and agency principles.
  • Assist staff in their duties when needed.
  • Design and implement services to meet participant needs.
  • Transports clients as necessary/appropriate.
  • Appropriately supervise all staff, students and volunteers.
  • Provide leadership, example instructions, suggestions, and discipline as appropriate.
  • Monitor client charts on a continual basis to insure that all treatment plans, progress notes and other materials are current.
Qualifications:
  1. Bachelor’s degree from an accredited institution in human services field or health and services field required.
  2. Minimum one year experience working with transition age participants required.
  3. Minimum one year supervisory experience required.
  4. Knowledge of children’s mental health systems is preferred.
  5. Knowledge of psychotropic medications, their uses.
  6. Comprehensive knowledge of methods of psychiatric rehabilitation required.
  7. Ability to make sound judgments regarding the health and welfare of clients required.
  8. Ability to work as a leader and provide training and supervision to staff required.
  9. Strong written and verbal communication skills required.
  10. Motivation, interest and ability to work empathetically with people who have disabilities (mental illness, physical, emotional, cognitive, etc.) required.
  11. Ability to work independently and effectively handle multiple assignments required.
  12. Valid driver’s license required.
  13. Willingness and ability to drive agency vehicle.
  14. Willingness and ability for on-call rotation required.
