Description

CareLink Community Support Services has a current opening for full-time, benefits eligible, Mobile Psychiatric Rehabilitation Specialist at our location in Springfield, PA. The Mobile Psychiatric Rehabilitation Specialist is responsible for providing skill and support development to consumers in their living, learning, working, and social environment within the framework of a service plan.
 

Available Shifts:

  • Monday through Friday, 8:30 AM – 4:30 PM


Salary Range:

  • $15.00/hour


Qualifications:

  • Bachelor’s degree from an accredited institution in social sciences field or related field and a minimum two years of mental health direct care experience required, which must include either 60 hours of psychiatric rehabilitation training or two years of work experience in psychiatric rehabilitation OR
  • High school diploma or equivalency and six years of mental health direct care experience, which must include either 60 hours of psychiatric rehabilitation training or two years of work experience in psychiatric rehabilitation.
  • Knowledge of psychotropic medications preferred.
  • Knowledge of basic principles and methods of rehabilitation required.
  • Motivation, interest and ability to work empathetically with people who have mental illness required.
  • Ability to work independently and effectively handle multiple tasks required.
  • Ability to work as a leader required.
  • Ability to make sound judgments regarding the health/welfare of consumers required.
  • Ability to handle confidential information required.
  • Valid driver’s license required.
  • Ability and willingness to drive agency car/van required.

 Essential Functions:

  1. Coordinate services and initial consumer contact with Case Manager at the anchor agencies.
  2. Provide skill development and support in a one-to-one relationship.
  3. Assist consumers in the development and implementation of long and short-term rehabilitation goals.
  4. Develop specific, achievable, behavioral-based and objective goals that address the skill areas the consumer has identified.
  5. Assist in the development of strategies to enable consumers to meet their rehabilitation goals.
  6. Provide direct care skills training in activities of daily living.
  7. Consistently assess, teach, evaluate and supervise consumers' skills in their living, learning, working, and social environments.
  8. Prepare service plans, progress notes, case summaries and other related paperwork in a timely manner.
  9. Organize schedule in order to maximize face to face contacts.
  10. Complete assignments independently and within specified time frames.
  11. Provide accurate and detailed tracking of service units.
  12. Demonstrate the ability to take direction.
  13. Follow instructions accurately and thoroughly.
  14. Adhere to supervisor’s requests.
  15. Show initiative and demonstrate good judgment.
  16. Demonstrate the ability to make sound judgments regarding the welfare of consumers.
  17. Work flexible schedule to accommodate program and consumer needs.
  18. Demonstrate knowledge of basic principles and methods of rehabilitation.
  19. Use appropriate communication channels.
  20. Display motivation and interest in working creatively with people who have mental illness.
  21. Perform the job within the framework of agency, site and professional standards.
  22. Participate in staff meetings and training programs.
  23. Participate in consumer meetings with other service providers.
  24. Participate in supervision on an on-going basis.
  25. Attend and participate in agency events.
  26. Adhere to agency/program policies and procedures.
  27. Project a positive attitude and image to staff, consumers, family members and outside contacts.
  28. Participate in achievement of agency/program objectives, goals and mission.
  29. Cooperate with other staff and exhibit positive attitude toward position and agency mission.
  30. Follow expected dress code.
  31. Transport consumers as necessary/appropriate.
  32. Respond appropriately to emergency situations.
  33. Provide service to a caseload of ten consumers.
  34. Performs job in a safe manner.
  35. Reports/addresses safety concerns in a timely manner.
